Pdf Summary
Beginning May 21, 2014, all DOT physical examinations for Commercial Driver's License (CDL) holders must be conducted by Certified Medical Examiners listed on the National Registry of Certified Medical Examiners. Employers must ensure their drivers meet the medical qualifications under FMCSA 49 CFR 391 regulations.

For drivers preparing for a DOT physical, it is critical to arrive on time and bring necessary documentation, including a company photo ID and current medical certification card. Drivers wearing prescription glasses or hearing aids should bring them to the exam. For those on blood thinners, proof of regulated INR levels for the last 30 days is necessary. Diabetic drivers must present recent A1C results under 10 that are less than six months old. Drivers with heart conditions should bring recent cardiac stress test results, while those who have had heart surgery need a cardiologist’s clearance letter. Drivers with pacemakers must provide documentation of annual checks.

Additional requirements exist for drivers with nervous or psychiatric disorders, seizures, or epilepsy, who must provide clearance letters from their healthcare providers. Drivers are also advised to document all prescription and over-the-counter medications they are using. For those on blood pressure medication or who register a blood pressure of over 140/90 during the exam, certification is limited to less than two years.

Drivers diagnosed with Obstructive Sleep Apnea require annual recertification by a sleep physician. They must demonstrate compliance with PAP treatment, using it for at least four hours per day on 70% of days, though optimal results show seven or more hours of nightly use. This clinic is open Monday through Friday to perform FMCSA physicals for CDL holders through certified medical examiners.
