So, today what we're going to do is we're looking at a skill prompt that looks around a key thing with the first rib, the cervical-thoracic junction, and this is looking at the area of the muscle with thoracic outlet syndrome. There are several key structures we're looking at with this, and that involves the clavicle, the pectoralis muscle, the muscle, and the scalene, and in testing it, we're using the ANSYS maneuver, two ways. One way, we're bringing the arm back, so we're going to feel the pulse, see if there's any changes in the muscle, numbness or tingling, which would indicate a problem with the brachial plexus, decreased pulse of the subclavian. The first method is we have the patient look towards us. We have them take a deep breath in, bring their chest out and hold that, see if we decrease the pulse or we get some of the other symptoms. The other way is have them turn their head away, and again, bring their chest out, deep breath in and hold it, and then feel, does it decrease the pulse, is there any symptoms associated with that, any numbness with that suggest impacting brachial plexus? And that's one way we can help test to look for thoracic outlet syndrome.
Video Summary
The video discusses a skill prompt focused on diagnosing thoracic outlet syndrome by examining key structures like the clavicle, pectoralis, and scalene muscles around the first rib and cervical-thoracic junction. Two methods of using the ANSYS maneuver are demonstrated. The first involves having the patient look towards the practitioner while taking a deep breath and extending the chest to check for decreased pulse or symptoms. The second method requires the patient to turn their head away while doing the same actions. Both approaches are used to identify brachial plexus issues by observing pulse changes and symptoms such as numbness or tingling.
