This presentation will provide an overview of the current Medicare payment policy landscape, outline the policies that have resulted in payment reductions in recent years, and describe the steps physicians can take to advocate for change. The presentation will also highlight some recent regulatory changes that present some opportunities for enhanced payment to office-based physicians, despite reductions to the Medicare conversion factor. 

After completing this activity, participants will be able to:
  • Describe the evolution of policy that has resulted in the stagnation of payment rates under the Medicare Physician Fee Schedule
  • Apply background of payment policies and their impact on the landscape of physician practices to advocacy to pursue reform
  • Identify recent policy changes that can support improved payment for office-based physicians in light of stagnant payment rates

Shawn M. Hamm, DO, MPH, is a dual board-certified Osteopathic Family Medicine Physician with UNC Health Blue Ridge and the Medical Director of his local health department, Burke County Public Health, in Western North Carolina. In addition to providing outpatient primary care for all ages, he serves as the Physician Liaison for Quality and Population Health for the UNC Health Blue Ridge Medical Group, focusing on strategies for improving quality metrics, reporting, and billing and coding throughout twelve practice locations. Prior to becoming a physician, Dr. Hamm served as the clinic manager of three federally-qualified health centers in rural North Florida, and later became a Performance Improvement Consultant and an Interim Bureau Chief at the Florida Department of Health. Dr. Hamm is currently the New Physician in Practice Member of the AOA Board of Trustees and the Chair of the AOA Bureau of Emerging Leaders.

Gabriel Miller currently leads regulatory affairs for the American Osteopathic Association. Prior to this role, he was a consultant with Avalere Health, supporting clients with policy analysis on a wide range of issues including ACA and commercial market insurance coverage, digital health issues, and physician payment policy. Prior to his time at Avalere, Gabriel was with AOA for four years working on regulatory affairs as a health policy manager.
