OVERVIEW
The way care is delivered is being fundamentally reimagined. Shared medical appointments (SMAs) and group-based care models offer a proven, scalable alternative to the traditional one-on-one encounter — enabling osteopathic physicians to meet patients where they are, address lifestyle factors in community, and extend their reach without sacrificing the whole-person approach at the heart of osteopathic medicine. This session explores how digital tools and tech-enabled care teams are amplifying that impact, from remote monitoring and patient engagement platforms to AI-assisted documentation and care coordination. Participants will leave with a practical framework for integrating these models into osteopathic practice settings.

  • Identify digital tools and platforms that support tech-enabled care teams in delivering lifestyle-centered, longitudinal care — including physical activity counseling, nutrition guidance, and behavioral health support
  • Apply a practical framework for integrating group visit models into osteopathic practice, with attention to workflow, reimbursement, and patient engagement considerations
  • Analyze how digitally supported team-based care structures can extend the osteopathic physician's capacity to facilitate the body's self-healing capacity across a broader patient population
